Clinical Education, Coordination and Nurse Navigation (CECaNN)
Program Guidelines
April 1, 2011 – March 31, 2012
Recipients must
reside in Butte, Colusa or Plumas counties.
be in need of services related to the breast cancer care continuum (outreach/ education, screening, diagnosis, treatment, follow up and recovery).
have an income 250% FPL or lower
Program Services
Ø Clinical and lay education, coordination and navigation services by a Registered Nurse and Care Coordinator Assistant to address barriers to quality care (financial, language/cultural, healthcare systems, travel/distance challenges, emotional barriers) and increase compliance with care regime for individuals in the breast cancer care continuum.
o The Nurse Navigator provides services such as:
§ Problem solving the healthcare system
§ Clinical education/clarification regarding
· Breast health
· Medical options
· Test results
· Questions to ask healthcare provider(s)
§ Decision support
§ Coordination of timely, standard care
· with the Primary Care Provider
· with the Referral Provider(s)
§ Emotional Support
§ Assistance obtaining financial resources for medical services, especially diagnostics
§ Arranging access to second opinions and clinical trials
o The Care Coordinator Assistant and/or Nurse Navigator provides services such as:
§ Assistance obtaining financial resources for ancillary services (transportation, child/elder care, purchase of personal items like head covers, prostheses, bras, lymphedema garments, food, housekeeping, utility payment, etc.)
§ Arranging for family/non family interpreters/translators
§ Arranging for one- on- one support from individual of same culture/language
Ø CECaNN staff provide assistance and support for community groups, service organizations, businesses and agencies interested in partnering to increase breast health awareness and access to screening mammography by planning and implementing a ‘screening mammography event’ at a mammography facility in any of the three counties.
