MSSP is a statewide program available in over 30 sites in California. The California Health Collaborative’s MSSP is a non-profit, community-based program that provides case management services to the frail and low-income seniors in Sacramento, Placer, and Yolo Counties. MSSP strives to keep seniors safely at home for as long as possible. The program is staffed by Registered Nurse Care Managers, Social Work Care Managers, and Case Aides who provide monthly monitoring and assistance.
 
 
Who is eligible for MSSP services?
 
A frail elderly individual who:
 
·   is 65 years of age or older
·   receives Medi-Cal benefits
·   is at risk of nursing home placement
·   needs human assistance to safely perform basic personal tasks, such as bathing,
    cooking, eating, toileting, and taking medications
 
Who can make a referral?
 
Anyone can make a MSSP referral. The following information is necessary at the time of referral: full legal name, address, social security number, date of birth, contact person, information about the situation, and the level of functioning for the person who is being referred. 
 
Is there a fee for services?
 
There is no charge for eligible MSSP clients. 
 
Other Helpful Information:
 
MSSP is a voluntary program whereby a client may cancel services at any time. A client may receive medical care through any health system. MSSP is funded by State and Federal sources through the California Department of Aging. Language interpreting services are available upon request.
