I am pleased to welcome you to the California Health Collaborative Website.
The Collaborative created this website to share critical information and valuable resources to visitors about our vision, mission, programs and services throughout many rural and urban communities in California.
The public health programs and services currently provided by the Collaborative represent strategic investments by our funders and our Board of Directors. I invite you to review the specific web pages containing information and resources for our health promotion and disease prevention programs that are serving the needs of many communities and underserved populations throughout the State of California.
Please consider us as a valuable agency for serving the public health and human service needs of your community, and if you would like to donate money, time or other resources to us, please use the information provided in the this website as to how and where to direct your contribution.

Who We Are
The California Health Collaborative (Collaborative) is a nonprofit 501(c)3 organization committed to enhancing the quality of life and health of the people of California, particularly the underserved and underrepresented. Founded in 1982 and headquartered in Fresno, California, the Collaborative has offices in Chico, Merced, Oakland, Sacramento, and San Bernardino serving residents in 54 out of the 58 California counties. Guided by its mission of "changing lives by improving health and wellness," the Collaborative implements an array of health promotion and disease prevention programs, public health surveillance systems, and a variety of capacity-building and networking activities.
The Collaborative has established strong partnerships at the local, regional, and statewide levels, enabling its programs and services to be linked into broader systems of prevention and care.

Through our partnerships, we deliver public health services in the following areas:
• Tobacco Control
• Maternal & Child Health
• Breast & Cervical Health
• Youth Development
• Nutrition & Physical Activity
• Cancer Surveillance
• Community Wellness
Responsiveness to health advocacy, patient access, and community education and outreach have long been hallmarks of the Collaborative. Since its inception the Collaborative has successfully implemented more than 50 programs, many of which have been regional or statewide in geographic scope. The Collaborative is committed to addressing the health needs of Californians, and places special emphasis on addressing the needs of the underserved, especially those who have limited access to resources affecting their well-being, and face barriers related to culture, language, income, education, gender, geography or immigration status.
For more than 25 years the Collaborative has helped to improve the lives of Californians by providing a variety of programs and services.

With offices throughout Central and Northern California, the Collaborative is uniquely positioned to provide services on a fee-for-service basis to public, private, and non-profit organizations. These services include:
• Grant and contract administration
• Cancer case-finding and data collection
• RN-provided case management/navigation support
• Public health program planning and evaluation
• Wellness programs
• Family life education (meets CA Department of Education code- compliance)
