The Patient Navigation and Support Project is operated through the California Health Collaborative, with funding from Susan G. Komen for the Cure Sacramento Valley Affiliate. Participation in the project is available to uninsured, low-income diagnosed breast cancer patients and to uninsured, low-income symptomatic individuals not eligible for CDP: EWC in the 14-county Sacramento Valley Affiliate services area.
